Job Description:

The Dietitian works as part of an interdisciplinary team to provide group and individual educational services related to nutrition and wellness. The Dietitian works collaboratively with providers and other health care teams to develop and deliver programs and services tailored to the individual or group needs. Serves as expert and resource within the organization in the use of food and nutrition to promote health and manage diseases. Prefer to have the dual certification.  If you could post it with CDE minimum and preference for LRD I think we might have better luck finding someone. 

Education Qualifications:

Key Responsibilities:

  • Identifies and assesses patient nutritional needs and creates care plans to address any identified risks. This may include but is not limited to working with critical care patients in the ICU, writing nutrition-related orders. related orders, providing appropriate nutrition education, assessing wound-related nutrition needs and completes timely and thorough documentation in the electronic medical record
  • Assesses for and diagnoses malnutrition
  • Educates on diet according to disease state, medical condition, and health risk
  • Coordinates discharge plans for patients returning home with nutritional support needs. This includes, but is not limited to, writing orders for home nutrition support equipment, creating and educating patient on nutrition support plans, navigating insurance plans for coverage of home nutrition support, and coordinating needs with the selected home medical supply company of patient’s choice
  • Acts as consultants to rural Essentia Health site providers for high acuity nutrition care concerns

CDE Requirement

  • Registered or registry-eligible with the Commission on Dietetic Registration of the American Dietetic Association within state(s) of employment. Must be registered within state of employment within six months of hire or transfer

  • Upon completion of 90 days of employment must apply for Advanced Practitioner Credentials through the Medical Executive Committee
