Job Description:

The Social Worker in collaboration with the hospice interdisciplinary team provides psychosocial interventions to all hospice patients/families/patient’s circle of support based on the biopsychosocial spiritual cultural assessment, patient/family’s goals-of-care and their acceptance of social work services promoting comfort, dignity, and well-being of the patient and their circle of support. •Anticipates and attends to patient’s specific psychosocial needs •Assists with end-of-life planning, and makes referrals to community resources •Provides education on end-of-life process •Assists with counseling around grief, family relationships, caregiver support needs, and emotional suffering related to end-of -life •Provides crisis interventions when needed •Assists with Advance Care Planning •Advocates and promotes patient’s comfort and dignity through the end of their life reflective of their culture and values •Provides care consistent with Medicare Conditions of Participation, Standards of Practice for Hospice Programs, and Essentia Health Hospice policies, and procedures •Master’s prepared social worker may provide supervision to bachelor’s prepared social workers •Master’s prepared social worker may provide education to hospice interdisciplinary team around psychosocial aspects present at the end-of-life

Education Qualifications:

  • A Master's degree in social work from an accredited social work (MSW) program

  • Current State Social Work Licensure

East Range Hospice:

  • Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency.
  • Proof of current motor vehicle insurance.

Duluth Hospice:

  • Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency.
  • Proof of current motor vehicle insurance.

Itasca Hospice:

  • Possession of a valid motor vehicle operator’s license as verified through appropriate licensing agency.
  • Proof of current motor vehicle insurance.
