Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients ranging in age from newborn to elderly as noted in age-related category and area of expertise.Education Qualifications:
- Full time position involving Heart Failure services.
- Clinic is Mon-Fri 8a-5p
- Treat clinic patients (10-13 per clinic day)
- Heart Failure clinic and follow-ups
- Outreach/telemedicine 2-3x per month in regional locations
- Work collaboratively on a team-based model that includes a mix of Cardiologists and Advanced Practice Clinicians and a strong team of nurses, techs and ancillary staff within our Heart & Vascular Department. Within the CHF clinic, you will partner with colleagues in managing the Heart Failure population across the continuum, resulting in some of the best outcomes in the nation.

Master's degree in nursing
Licensure/Certification Qualifications:
Current licensure as a registered nurse in appropriate state.
Current licensure as a Nurse Practitioner in appropriate state.
Current certification in CPR.
National certification in applicable area.
Successful completion of Essentia Health credentialing process prior to practice
