Job Description:
Responsible to provide leadership, program development and coordination between providers, staff and patients for a defined population. Acts as a primary liaison between the operations team, internal stakeholders and key community partners. In conjunction with internal departments and disciplines, coordinates community outreach programs, including screening and health promotion activities, and education. The position may include direct patient care in the role of care coordination for a defined population. Ongoing collaboration with providers, staff, quality, regulatory and reimbursing agencies are required for optimal care coordination across the continuum. Position requires knowledge and ability to research evidence-based practices analyze and interpret data to make program recommendations and improve population outcomes.Education Qualifications:
Required Education:
- Bachelors degree in Community Health, Business or Healthcare related field
Required Experience:
- 3 years healthcare experience is required
Preferred Qualifications:
Prior experience with home and community care based services, and health & wellness or chronic disease management preferred.

Hybrid Nature:
- This position will include monthly travel within our health system (Minnesota, North Dakota, Wisconsin)
- Candidates must sit within our service area and be able to come onsite
- Most of this work will be conducted remotely
