Responsible for organizing and providing nursing care to patients through the process of assessment, planning, intervention, and evaluation. Delegates aspects of care to other nursing personnel based upon their licensure, preparation and job descriptions. Contributes to the meeting of the mission and goals of the facility and Essentia, and meets the requirements of the Joint Commission if applicable and/or other federal, state and local regulatory or accrediting agencies. Responsible for care coordination, patient education, and decision-making support for patients. Serves as a single point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Essentia and in the community. Participates in conferences and committees, assisting the entire care team in developing optimal care plans for each individual. Collaborates with other service providers/organizations and community supporters in providing patient education and outreach.For staff working in Pap Navigator RN team Under Quality Department:RN responsible for monitoring cervical cancer screening results and implementing guidelines for management with provider supervision and direction. Job responsibilities may be performed off-site (a non-Essentia health site).For staff working in Lung Navigator RN team Under Quality Department:RN responsible for monitoring Lung Nodule and Incidental screening results and implementing guidelines for management with provider supervision and direction. Job responsibilities may be performed off-site (a non-Essentia health site).Education Qualifications:
Educational Requirements:
- BSN or ADN degree from an accredited school or college of nursing
Required Qualifications:
- 2 years of related experience
Licensure/Certification Qualifications:
Required Certification/Licensure:
- Current nursing licensure in state(s) of employment
- Basic Cardiac Life Support (BCLS) certification within 1 month
East Oncology Department:
- Oncology Certification Nurse (OCN) within 2 years of hire
- Certified Pediatric Hematology Oncology Nurse (CPHON) required for those serving pediatric patients within two years of date of hire (includes department 11160)
For staff working under department 1000-46490:
- For staff hired on or after August 1st, 2022: Must hold a current license in Professional Nursing from one of the three states upon hire: Minnesota, Wisconsin, or North Dakota. Must earn licensure in the remaining two states within 90days of date of hire or transfer
